HIPAA PRIVACY NOTICE
Nassau Hematology Oncology, P.C. (NHO) is committed to providing patients with
high quality medical services and to provide these services pursuant to the highest
ethical, business, and legal standards. We are required by law to inform our patients
of information relative to their protected health information rights. You can
view a copy of the HIPAA Privacy Notice.
NHO is also required by HIPAA to obtain a written
consent from each patient prior to using or disclosing his/her health information
for treatment, payment and health care operations. Kindly sign the written consent
and present it to the office during your consultation.
EDUCATION
NHO is equally committed to educating each patient during the course of treatment.
In our field some treatments require chemotherapy; our oncology certified nurses

for patients undergoing chemotherapy.
